Time includes 15 minutes for check-in and check-out.  Must have previously scheduled a First Office Visit.  If we have not seen you for 3 years please call our office and schedule a First Office Visit.  If we are billing insurance for you, please bring your card if your insurance has changed.  The office is located on the 2nd floor and accessible by following the south side fence, going through the gate, then up the stairs.  If you cannot climb stairs please let us know so we can make accommodations.  Price quoted is the minimum cost for a 45 Minute Follow-up Visit, and must be paid for at the time of service, unless you have in-network insurance coverage for our services.  Actual cost may be more depending on the extent of services rendered.
